LIFETIME CARE AND SUPPORT AND SERIOUS INJURY CASE MANAGEMENT SERVICES
We have case mangers that are accustomed to working with participants and their families as they progress through life post injury.
Services can commence in preparation of:
- Hospital discharge
- Completion of services including community based rehabilitation
- Assistance with identification of appropriate services and submission of associated plans
This means that as your needs change our case management services will change with you. This enables you to ‘test the waters’ and decide what works for you.
OCCUPATIONAL THERAPY SERVICES AND ASSESSMENT
Our occupational therapists provide assessment and therapy services across NSW and nationally by negotiation.
Some examples of services Occupational Therapists may complete include:
- Accessing public transport
- Meal preparation
- Planning and time management
- Becoming independent with showering or personal care
- Reducing the need for attendant care worker support
- Equipment needed to take a holiday
